Q: Do you take insurance?

A: Endurance Sports Physical Therapy & Performance, LLC is a direct pay, out-of-network physical therapy provider. We will not bill your insurance company directly. However, upon request you will be issued proof of payment and documentation regarding your visit that you may submit to your company for reimbursement. Reimbursement is not guaranteed, and is most often consistent with the out-of-network provider rate. Physical therapy services can be paid for using your health savings or flex spending accounts (HSA/FSA).

Q: Do I need a referral or order from my doctor?

A: No. Minnesota is a direct access state, meaning you do not need a referral/order from another health care provider to see a physical therapist. This allows for greater cost savings and earlier access for you.

Some insurance plans do require a referral/order if you wish to submit your care for reimbursement. In this case, you will need a referral prior to your initial visit.

Q: What type of payment do you accept?

A: Accepted forms of payment include: Flexible Spending Accounts (FSA), Health Savings Accounts (HSA), credit or debit card, check, or cash at the time of service. Please note that FSA/HSA can only be used for physical therapy services.

Q: Can I schedule appointments online?

A: Yes! You can do so here.

Q: Do you offer gift cards?

A: Yes! Gift cards can be used for all services and can be purchased online here.

Q: What if I don’t see a day or time that works for me?

A: We strive to make early access for all patients a priority. If you are unable to schedule an appointment on a day or time that doesn’t work for you, please let us know. We will do everything we can to work you into our schedule.

Q: How long after an injury should I wait to get treatment?

A: Generally, the longer you wait to get treatment, the longer it will take to return to full and pain-free health. In many instances, this may require only 1-3 treatment visits for acute injuries (< 3 months since onset).

Q: How long are physical therapy visits?

A: Most initial and follow-up visits are scheduled for 45 minutes. However, because we do not work with insurance companies directly, we can easily adjust the length of the visit to meet your needs. This will be discussed at your initial visit.

Q: What do I need for my first visit?

A: Athletic clothing that will allow for comfortable movement. Please bring your current athletic shoes as well.

Q: How many visits will I need to get better?

A: This is a difficult question to answer generically, and depends on many factors. At your initial visit, Michael will discuss the prognosis for your condition, and frequency/duration of your future visits will be determined collaboratively to meet your needs. Generally, most patients require one visit/week for 3-6 weeks until their pain and symptoms resolve.

Q: What can I expect in the initial evaluation

A: Initial visits begin with a complete review of your relevant medical history, current injury, and goals for treatment. Following a comprehensive examination, you can expect to learn about your diagnosis, including why the injury occurred, the root cause of the problem, and how physical therapy can help. From there, we formulate a plan to achieve and maintain your desired activities pain-free. Treatment is initiated, and you will be given detailed instructions, photos, and videos on how to perform corrective exercises outside of physical therapy. You will leave knowing how long it will and what steps are required to achieve your desired outcome.

Q: Will I need to fill out any paperwork prior to my first visit?

A: Yes. Paperwork and forms will be provided once scheduling is complete.
